Two New Services Offered at Inova Loudoun Hospital
Two clinics celebrated grand openings Monday.
Inova Loudoun Hospital celebrated two of its newest services Monday during a grand opening event of two new clinics – the Inova Discharge Planning Clinic and the Inova Juniper Program, which are located in the Jackson Building at Inova Loudoun Medical Campus.
The Inova Discharge Planning Clinics were created to serve chronic diseases such as diabetes, chronic obstructive pulmonary diseases or COPD/pneumonia, congestive heart failure or CHF and asthma. The vision for these clinics is to help transition patients who had recently been released from the hospital back into the community while preventing hospital readmissions, according to a news release.
The new clinic will provide primary medical care, medication reconciliation and social work interventions, among other things.
The Inova Juniper Program, Northern Virginia's largest provider of HIV/AIDS care, will now be able to provide primary medical care, pregnancy management, nutrition counseling and other services to those living within the local community.
The program has provided care to more than 1,300 persons living with HIV/AIDS throughout Northern Virginia.
“We are committed to serving the needs of all those in our community,” said H. Patrick Walters, CEO, Inova Loudoun Hospital. “Providing these new services will increase accessibility to care and enhance health outcomes for those without a medical home and those living with HIV,” Walters said.
